I had a 12 inch RBTA decide to crawl into my Koralia 3. The only thing left was a small piece of flesh blowing out the powerhead. The rest was minced and in my skimmer. I thought the regular skimmate smelled bad! None of my livestock showed any signs of stress over its death though.
All my current BTA's don't wonder into the powerheads for some reason. They will wonder within inches of them, but not into them. I guess I'm lucky!
