how well did you acclimate it to salinity? they can be very sensitive. That being said once accimated to the tank they are usually very hardy as well. They're difficult to get into as well so I doubt...

They don't look like they could fit a small fish in their mouths but they have a hinge-ing system to their jaw, like a grouper. Their mouth gets pretty huge and they'll cram whatever fits into it...

Head and lateral line erosion. Some blame it on excess carbon or carbon particles in the water. It can also be caused by poor nutrition I think.
