Default Any experience with CITES permit for personal coral from USA?

As thread title, wondering if anyone has experience with CITES permitting? I have a specific coral in mind from a vendor so I have that nailed down. It doesn't look like there is a cost for this either?

Looks like all types of mushrooms are exempt as long as there is no LR.

Questions on the application:

For genealogy of live animal, do you need to fill out? I suppose I could email the CITES management office to ask as well.

I'm not the type to make a border run and hope for the best lol, get too nervous. Any tips/help that can be provided would be wonderful.
