I wouldn't let the nitrates become detectable but I would slowly increase feedings until nitrates start to become detectable or you get a small algae bloom and then back off on the amount a little...

Hard to tell without seeing the fish, but i would recommend setting up a hospital tank asap and putting all the fish from the main tank into it to treat. It could be ich but there are other diseases...

I quarantined mine in a 4ft, 55g tank with 2 hagen 802 powerheads and 2 maxijet 1200's. He had marine velvet when he arrived and had to be treated with copper but he made it through the 8 weeks...
