In order to earn a Co-op credit for this work experience, an Employment Consultant must approve the position. If you have obtained your own job, please complete this form with your employer and submit it to your Employment Consultant, prior to commencing the work term.

An on-site monitoring visit by a Consultant and two employer evaluations (Interim, Final) of the student are integral components of the Co-op process. The Consultant will call to arrange an appointment and the student will provide the evaluation forms to the employer. The employment detailed above is eligible as a Co-op credit, provided the student satisfactorily completes the assignments required by the Job Centre.
