You could shake the rock or wave your hand in the water in front of them to get them to close up and use a hammer and chisel to chip off a small peice with them on it so you have something to secure them with on the new rock. Or just pull the rock out. They'll close up.

Just take a pair of frag cutters and cut at the rock just below them and then glue to a new one.

How long can it be out of the water while I perform the surgery?

as long as you think most corals are more tolerant of the air then we think, if its out too long just keep it wet, shouldn't take you more than like 5 min anyway

if its gonna be long get a Tupperware container fill it with salt and do the " surgery" under water
