Since I got a tank crash with Kent carbon I have discontinued the use of carbon and I am very afraid of ever using carbon again.
It is hard to tell if there are contaminants or not to begin with so I prefer to stay clear of it.
As for wether carbon can release bad stuff in the tank, I got my answer when my reef was contaminated with copper contained in the carbon, even after a very good and long rinsing under running water. So yeah, I do beleive carbon can release back into the tank.
